Injectable testosterone cypionate is a popular, reliable form of testosterone replacement therapy (TRT). One common question we hear from men on TRT is what size needle should they use, and does the size of the needle matter?
According to Dr. Jordan Glenn of Thunderbird Men's Health, the answer often comes down to balancing comfort with practicality.
Smaller Needles (30–31 Gauge)
Very small needles, such as 30- or 31-gauge insulin syringes, are often used for subcutaneous injections. And while they may seem less intimidating than larger-gauge ones, they can create challenges when injecting testosterone cypionate.

Testosterone is suspended in an oil-based solution like sesame or grape seed, which is much thicker than insulin. Because of the tiny needle diameter, drawing the medication from the vial can be slow and difficult. Injecting the medication can also take significantly longer and require more pressure.
Larger Needles (23–25 Gauge)

Dr. Glenn generally recommends a bigger, 23- to 25-gauge needle for most TRT patients. Although these needles appear larger, they offer several advantages:
- Easier to draw testosterone from the vial
- Faster, smoother injections
- Less pressure required during administration
- Shorter overall injection time
"A lot of patients assume the smaller needle is better," says Dr. Glenn. "But when you're giving yourself an injection, you don't want the needle in any longer than necessary."
Which Needle Should You Use?
For most men using testosterone cypionate, a 23- or 25-gauge needle provides the best balance of comfort and convenience. It works well for both drawing up the medication and administering the injection, whether prescribed for intramuscular or subcutaneous use.
